If you’re a provisional psychologist completing the 5+1 Internship Program, or a supervisor supporting 5+1 Interns, there are important updates you need to be aware of.

These changes relate to the official documentation required by the Psychology Board of Australia and administered under Ahpra. Using the correct forms is essential to ensure your internship remains compliant and progresses without unnecessary delays.

New Forms Effective 1 December 2025

The updated 5+1 Internship forms came into effect from 1 December 2025. All interns and supervisors should ensure they are using the revised versions from this date onward.

Using outdated documentation may result in processing delays or requests to resubmit information, so it’s important to download the most recent versions directly from the Board’s website before completing any submissions.

Below is a summary of each updated document and its purpose within the 5+1 Internship Program.

INPP-76 – Internship Program Plan (effective from 11 November 2025)

Purpose:
The INPP-76 form outlines the structure of the internship before it begins. It details the proposed supervision arrangements, learning objectives, areas of practice, and how the internship will meet the required competencies.

This form must be completed and submitted to the Board prior to commencing the internship. It demonstrates that the planned program meets regulatory requirements and provides a clear roadmap for both the intern and supervisor throughout the year. Use this form to gain approval for your work role under the internship upon commencement, when adding or changing a work role.

 

CHPS-76 – Change of Principal Supervisor (effective from 1 December 2025)

Purpose:
The CHPS-76 form is used when an intern changes their principal supervisor during the internship year.

It ensures the Board is formally notified of the change and that the new supervision arrangements continue to meet approval standards. This form supports continuity of internship progress and confirms that supervision responsibilities are appropriately transferred and documented.

 LBPP-76 – Log Book

Purpose:
The LBPP-76 logbook is used to formally record internship hours, including psychological practice, supervision, and education and training activities.

It provides a structured record of the intern’s progress toward the required total hours. Accurate completion of the logbook is essential, as it forms part of the evidence reviewed when applying for general registration and should be reviewed weekly, at minimum fortnightly by the principal supervisor.

Important Note About the Internship Logbook (LBPP-76)

 

 PACF-76 – Final Assessment of Competence (effective from 1 December 2025)

Purpose:
The PACF-76 form is completed at the conclusion of the internship and documents the supervisor’s formal assessment of the intern’s competence.

It evaluates whether the intern has met the required competency standards for general registration. This is a critical document, as it supports the intern’s application to transition from provisional to general registration.

 5+1 Internship Program – Progress Review Template (Word)

Purpose:
The Progress Review Template is used to document structured reviews conducted throughout the internship in 6 monthly cycles.

Regular progress reviews provide an opportunity to reflect on competency development, identify strengths and areas for growth, and adjust the learning plan if required and review the supervision to practice ratio. These reviews support transparent communication between supervisor and intern and provide documented evidence of developmental progression across the year.

This template is a new form to compliment the transition from progress reports to progress reviews within the 5+1 internship.

 

What You Should Do Now

  • Download and review the updated forms.
  • Ensure all new submissions from 1 December 2025 onwards use the revised documentation.
  • Continue using the existing logbook template until the updated Excel version is released.
  • Monitor for further updates regarding the revised logbook.

Staying across these changes will help ensure your 5+1 Internship proceeds smoothly and remains fully aligned with Board requirements.
