5+1 Internship - Important Update

If you’re a provisional psychologist completing the 5+1 Internship Program, or a supervisor supporting 5+1 Interns, there are some important updates you need to be aware of.
New Forms Effective December 1 2025
The updated 5+1 Internship forms came into effect from December 1, 2025. All interns and supervisors should ensure they are using the revised documentation from this date onward to remain compliant with program requirements.
Important Note: At this stage, we are still awaiting the release of the updated Excel logbook that aligns with the new forms.
We have made enquiries regarding the expected release date and will provide an update as soon as further information becomes available. In the meantime, please continue using the current logbook until the revised version is officially published.
New Forms Effective December 1 2025
- INPP-76 - Internship program plan (789KB, PDF) – effective from 11 November 2025
- CHPS-76 - Change of principal supervisor (769KB, PDF) – effective from 1 December 2025
- LBPP-76 - Log book (63.2KB, PDF) – effective from 1 December 2025
- PACF-76 - Final assessment of competence (770KB, PDF) – effective from 1 December 2025
